1 Day Permit: $5.00
3 Day Permit: $9.00
Please bring a valid photo I.D. Any document to be notarized must be filled out. Please leave signature blank and sign in front of notary.
Notary: $2.00 per signature
Black and white copy services are available. Small quantities of copies are $0.25 per page. Cost for large copies determined upon time of service.

PUBLIC RECORDS REQUEST
A records request form must be filled out for any public records to be inspected or copied. Record request forms are available at the link below or at PRFMD's administration building. Please attach any applicable documents (for medical records only) to the records request form.
To submit a request please mail to our administrative office, or send via email to firstname.lastname@example.org. We do not accept requests via fax.
Please allow 15 business days to process your request. If it is an extensive request it make take longer than 15 business days and PRFMD will notify you of estimated processing time.
For Medical Record Requests: Patients requesting medical records must provide proof of identification (government issued I.D.). Third parties requesting a patient’s medical record must attach one of the following to the Records Request Form: (1) a notarized HIPAA-compliant release (see 45 C.F.R. §165.508 for federally required contents of release) signed by the patient; or (2) a court order signed by a judge authorizing release (45 C.F.R. §164.512). A subpoena without a HIPAA-compliant release or court order is not sufficient. For questions: Call (520) 682-7878. In order to protect patient privacy PRFMD does not email reports that contain a patient’s protected health information.

BURN PERMIT REGULATIONS
- Only natural vegetation you have cleared from your property may be burned.
- Plywood, 2x4's etc., any treated wood, trash or animal waste MAY NOT BE BURNED. See below for list of prohibited materials.
- Property and piles must be inspected by fire district personnel prior to burn permit being issued. Piles may not exceed 4'x4'. Material may not exceed 2" in diameter. Burn pile must be 50' from any structure.
- Permits may be rescheduled only when canceled by the fire district for weather or extreme fire danger.

BURNING THAT CREATES A NUISANCE OR GENERATES NOXIOUS FUMES IS PROHIBITED. ADDITIONALLY, BURNING ANY OF THE FOLLOWING IS PROHIBITED:
Food packaging and serving materials, specified plant materials (oleander, poison oak, poison ivy, and poison sumac), petroleum products and/or their containers, plastics and other synthetic materials, all construction materials, and known hazardous materials and/or their containers.
